Structural Brain Correlates of Bulimia Nervosa Diagnosis and Symptom Severity: A Coordinated ENIGMA Eating Disorders Working Group Analysis

Berner LA, Phadnis A, Westwater ML, et al. ยท JAMA Psychiatry ยท September 2026

This ENIGMA Eating Disorders Working Group analysis pooled structural MRI data from 786 women across 17 international cohorts โ€” 369 with bulimia nervosa and 417 healthy controls โ€” the largest neuroimaging study of BN to date. Compared with controls, women with BN had smaller nucleus accumbens volume (Cohen d = โˆ’0.20) and reduced cortical surface area in the superior and transverse temporal cortices (d โ‰ˆ โˆ’0.24 to โˆ’0.26), regions tied to reward valuation and to auditory/social processing.

More frequent binge eating tracked with broader surface-area reductions across reward, cognitive-control, interoceptive, and social-cognition circuits. The differences held after adjusting for BMI and depression, arguing against a purely nutritional or mood-driven explanation.

๐Ÿ’ก Why it matters

The reward-circuit findings back a framing many residents already use intuitively โ€” BN behaves as much like a reward-dysregulation disorder as a weight/shape disorder, worth naming explicitly when explaining treatment rationale to patients.

Treatment of Depression in Patients With Alcohol or Other Drug Dependence: A Meta-Analysis

Edward V. Nunes, Frances R. Levin ยท JAMA ยท April 2004

This meta-analysis pooled 14 randomized placebo-controlled trials of antidepressants (mostly TCAs and SSRIs) in patients with major depression and comorbid alcohol or drug dependence โ€” the study that first quantified whether it's worth treating mood pharmacologically in patients still using or recently sober. The pooled effect size for depression was a modest 0.38, smaller than antidepressant effects typically seen in depression alone; the effect on substance-use outcomes was smaller still, at 0.25.

But the two effects were tightly linked: in trials where the antidepressant effect on mood reached medium size (โ‰ฅ0.50), the paired substance-use effect size rose to 0.56 โ€” treating the mood disorder often moved the addiction too. Trials that paired medication with psychosocial addiction treatment outperformed medication alone.

๐Ÿ’ก Why it matters

Two decades on, this is still the working justification for not withholding antidepressants until a patient achieves sobriety โ€” expect a modest but real effect, and don't expect medication alone to substitute for addiction treatment.

Carbamazepine's Autoinduction Curve Sets Your Titration Schedule, Not Just Your Target Dose

Carbamazepine induces its own metabolism through CYP3A4, and the process isn't instant โ€” it unfolds over roughly three to five weeks, during which a fixed dose's serum level can fall by 30 to 50 percent. A level drawn in week one that looks therapeutic can be subtherapeutic by week five on the same dose, which is why carbamazepine needs levels timed around the induction curve, not just at steady state.

The kinetics reverse too: after roughly a week off carbamazepine, autoinduction partially resolves, so restarting at the prior maintenance dose can produce levels high enough to cause ataxia or diplopia โ€” restart lower and re-titrate rather than resuming where the patient left off.

Autoinduction is also behind carbamazepine's most consequential interaction: it lowers estrogen and progestin levels in combined oral contraceptives enough to cause breakthrough ovulation, so starting carbamazepine on hormonal contraception means a backup method, not just a counseling note. The same CYP3A4 induction accelerates clearance of other 3A4 substrates โ€” several benzodiazepines, some antipsychotics, warfarin.

A Role Dispute's Stage โ€” Not Its Content โ€” Tells You Which Technique to Use

IPT's role-dispute framework isn't about mediating who's right in a conflict โ€” it's about locating which of three stages the dispute has reached, because each stage calls for a different technical move. In renegotiation, both parties still believe the relationship can change; the work is communication analysis and option-generation, helping the patient voice wants directly instead of through resentment.

In impasse, both sides have stopped trying and low-grade conflict has become the relationship's steady state; here the therapist's job is to reintroduce the possibility of change before any negotiation content โ€” naming the impasse itself as the problem. In dissolution, one or both parties has already decided the relationship is ending, and the work shifts to grief, not repair.

Applying renegotiation technique to a dispute that's actually at impasse โ€” pushing communication skills before the stalemate is named โ€” is a common resident error, and it stalls.

๐Ÿ—’๏ธ Vignette

A PGY-3's outpatient, married eight years, opens session after session with the same complaint about her husband's silence, then pivots to logistics. Early sessions used standard communication-analysis technique โ€” reviewing a recent argument line by line โ€” with no movement; she reported the identical fight two weeks later, unchanged.

Supervision reframed the read: this wasn't a renegotiation-stage couple still generating options, it was impasse โ€” years of the same unresolved argument had calcified into a stable, low-grade standoff neither person expected to end. The next session, the resident named that pattern directly: "It sounds like you've both stopped expecting this to change." The patient paused, then said that was the first time anyone had said it out loud โ€” and named, for the first time, that she'd been considering separating.

โ—† A JAMA Network Open analysis of 306 state budgets (2021โ€“2026) found per-capita suicide-prevention and crisis-service funding rose roughly sixfold from 2021 to a 2025 peak before dipping in 2026 โ€” states with an enacted 988 telecom fee funded crisis services at more than double the rate of states without one, evidence worth citing when advocating locally for that funding mechanism.  source โ†’
