Brain lithium temporospatial kinetics in bipolar disorder: a repeated-measures 7Li MRI study of dosing regimens in Germany
Ritter P, Edelmann K, Thelwall PE, Boumezbeur F, Bauer M, Werner A, Hall G, Kavanagh O, Cousins DA ยท Lancet Psychiatry ยท August 2026 (online June 28, 2026)
Once-daily and twice-daily lithium work about equally well, but they produce very different serum curves across the day, and nobody has known whether the brain smooths those peaks out. This group used lithium-7 MRI, which directly images lithium in brain tissue, in 41 euthymic outpatients with bipolar I or II on stable lithium (20 on once-daily evening dosing, 21 on twice-daily). Each patient had three brain scans with matched serum levels across a 10-hour day (0800, 1400, 1800).
At the standard 12-hour post-dose timepoint, brain signal and serum levels were equivalent between regimens. After that the curves diverged: on once-daily dosing serum lithium fell across the day, while on twice-daily it rose after the morning dose and then declined, and brain lithium closely mirrored serum in both groups rather than being buffered. Lithium signal was higher in white than grey matter, and grey matter equilibrated with serum faster than white matter or CSF.

๐ก Why it matters
A 12-hour trough means the same thing on either regimen, so one reference range is defensible, but a level drawn at any other time is not comparable between regimens. Because the brain tracks serum rather than smoothing it, timing of the draw matters more than many residents assume, and a once-daily regimen really does give the brain a daily trough. |

Treatment dose and reliable change in trauma-focused therapy for comorbid PTSD and MDD
Kline AC, Crocker LD, Otis NP, Vera JD, Myers HC, Glassman LH, Hunt WM, Walter KH ยท Psychotherapy Research ยท September 9, 2026 (online)
Depression alongside PTSD predicts poorer response to trauma-focused therapy, but little is known about how much therapy that comorbid group actually needs. This secondary analysis followed 88 service members with both PTSD and major depression who were randomized to cognitive processing therapy (CPT) alone or behavioral activation followed by CPT (BA + CPT) in a clinical trial (NCT02874131). Survival analyses tracked when patients achieved reliable change in PTSD, depression, or both.
Among patients who reached reliable change, median attendance was the full protocol, and reliable change was more likely with completion than with a merely "adequate" dose. Reliable change in both disorders came more slowly in the integrated arm (hazard ratio 0.52, 95% CI 0.28 to 0.97), though differences for PTSD or depression alone were not significant.

๐ก Why it matters
For patients with PTSD plus depression, the benefit comes late in the protocol, so treat an early dropout conversation as a clinical event, not an administrative one. Front-loading behavioral activation did not speed things up and may have pushed dual response later. |

Aripiprazole lowers prolactin by occupying the receptor, not by blocking it harder
When risperidone or paliperidone drives prolactin up, adding low-dose aripiprazole usually works better than switching or reaching for a dopamine agonist. The mechanism matters: pituitary lactotrophs sit outside the blood-brain barrier, and risperidone and paliperidone are P-glycoprotein substrates that reach higher D2 occupancy at the pituitary than in the striatum. Aripiprazole's very high D2 affinity lets it displace the antagonist, and its partial agonism provides enough dopaminergic tone to suppress prolactin release. A 2021 network meta-analysis of 62 RCTs found adjunctive aripiprazole at under 5 mg/day produced the largest prolactin reduction of any adjunct, so start low; more is not better for this indication. Bromocriptine and cabergoline also lower prolactin but carry a real risk of destabilizing psychosis. Before treating, confirm the elevation is drug-induced: repeat a resting sample and rule out pregnancy, hypothyroidism, renal failure and macroprolactin, and image the pituitary if levels are far above what the drug usually produces.

Interpret in displacement before you interpret in the transference
A transference interpretation is only useful if the patient can hear it. For patients with fragile self-esteem or a quick shame response, "you feel I'm not paying attention to you" can feel like an accusation, and the result is compliance or withdrawal rather than insight. Interpretation in displacement addresses the same conflict through the figure the patient is already talking about: the boss, the partner, the character in a dream. The patient keeps a safe distance, the therapist tests whether the idea lands, and a shared vocabulary builds that can later be carried into the room. This is not avoiding the transference. It follows the principle of working from surface to depth, and it treats the patient's choice of displaced figure as information about how close to the therapist the feeling can be allowed to come. The move to a direct transference link is timed to evidence that the patient can tolerate it: they begin drawing the parallels themselves.

๐๏ธ Vignette
A 34-year-old engineer spends a session angry at a supervisor who "only notices the people who complain." Two weeks earlier the resident had cut the session short for a page, and the patient said it was "totally fine." The resident thinks of the link but judges that a direct comment would feel like being caught. Instead: "It sounds painful to work hard and quietly and then watch attention go to whoever makes noise, and maybe hard to say so without feeling like you're the one making the fuss." The patient agrees and adds that he never asks for anything. Three sessions later he mentions the shortened hour on his own, half-joking. Now the resident makes the link: "Maybe it wasn't totally fine here either, and saying so felt like becoming the person who complains." The displacement prepared the ground, so the transference interpretation arrives as recognition rather than accusation. |
